The Transitional Rehabilitation Housing Program (TRHP) provides intensive case management services to forensic clients, promoting recovery and helping them to successfully navigate the transition into community life.
This is accomplished through focusing on skill development, psychosocial rehabilitation and recovery, Case Managers support clients in securing and maintaining appropriate housing, linking clients with community services, and supporting them in adhering to the conditions of their dispositions, which are legal orders issued yearly by the Ontario Review Board.
TRHP clients are supported in independent units with intermittent staff support.
This is a partnership between CMHA-Thunder Bay Branch and TBRHSC-Forensic Unit.

Eligibility
Persons 18 & over who are residing in the Forensics Unit at TBRHSC with serious mental health issues and have been found Not Criminally Responsible.
Referral Sources
Referrals come directly from Thunder Bay Regional Health Sciences Centre-Forensic Unit.

Monday – Friday, 8:30 a.m. – 4:30 p.m.
