The Housing First model is a minimal barrier housing service specifically designed for individuals living with concurrent mental illness, addiction who have experienced chronic homelessness. This program understands the challenges that people have experienced and works collaboratively with the client to address areas of concern in a strength based, recovery focused model.
The Housing First Case Manager will help the client make connections in the community regarding food security, mental health support, access to primary care and addictions treatment. They will liaison between the landlord and the client and the program offers a financial rent supplement component for eligible applicants. The case manager will assist with promoting independence and teaches life skills in the home environment with the client.

Eligibility
Persons 18 & over with a mental health issue or concern. Chronically homeless for more than one year.
Referrals for the Housing First program come from a wide variety of community partners including the Shelter House, the Salvation Army, Grace Place, mental health agencies, the custody facilities, the health care system, etc.
Hours of Operation
Monday – Friday, 8:30 a.m. – 4:30 p.m.
