FSHP: Forensic Supportive Housing Program

The Forensic Supportive Housing Program provides intensive case management services to forensic clients who are admitted to the hospital, promoting recovery and helping them to successfully navigate the transition into community life from the Hospital setting. This program will assist people when they are residing in the transitional unit and assist them when they are ready to graduate into living independently in the community.
In collaboration with the hospital staff, the FSH program goals are accomplished through focusing on skill development, psychosocial rehabilitation and recovery. Case Managers support clients in securing and maintaining appropriate housing, linking clients with community services, and supporting them in adhering to the conditions of their dispositions, which are legal orders issued yearly by the Ontario Review Board.
FSH clients are supported in independent units with intermittent staff support and liaison between community partners and the landlord. FSH Case Managers provide in person support to clients in their unit which help assist with their success such as cooking, cleaning, pro social recreational activities, positive coping skills, connection to primary care appointments and managing symptoms of mental health at home.
This is a partnership between CMHA-Thunder Bay Branch and TBRHSC-Forensic Unit.

Eligibility
Persons 18 & over who are residing in the Forensics Unit at TBRHSC with serious mental health issues and have been found Not Criminally Responsible.
Referral Sources
Referrals come directly from Thunder Bay Regional Health Sciences Centre-Forensic Unit.
Hours of Operation
7 days a week with a flexible schedule.
